Celebrate Gymnastics Forms and Information
In order to participate in any of our programs, every gymnast must have a current registration form and waiver on file. Please download, print, and fill out these forms then return them to us at the start of your child's first class. If you are a parent or guardian particpating in our Parent-Tot classes, please also fill out the adult waiver. Thank you!
